Obesity is a chronic, multifactorial condition — not simply a matter of discipline. Effective therapy combines nutrition, exercise, behavioural support and — where indicated — medicinal components under medical supervision. The indication for medicinal support is set individually based on BMI, comorbidities and risk profile.

Hunger and satiety are hormonally regulated — in many people with overweight this system is shifted. Modern medicinal options act here: GLP-1 receptor agonists (active substances semaglutide and liraglutide) and the combined GLP-1/GIP agonist tirzepatide reinforce the body's own satiety signals, slow gastric emptying and — when used correctly — can enable meaningful weight reduction.
Three active substances are available in Austria: semaglutide and liraglutide (GLP-1 receptor agonists — approved both for type 2 diabetes mellitus and, in higher dosing, for obesity therapy) and tirzepatide (combined GLP-1/GIP agonist, approved for type 2 diabetes and for weight management). Which substance fits in an individual case is decided after diagnostics and informed consent on effect, possible side effects and contraindications.
The injection alone is not the solution. Sustainable weight reduction emerges from the interplay with nutrition, exercise, sleep and behavioural strategies — this support is part of the therapy and is structured in follow-up appointments. When discontinuing the medication without lifestyle adjustment, partial weight regain is to be expected.
Treatment building blocks

When medical support is useful
- Obesity (BMI ≥ 30 kg/m²)
- Overweight (BMI ≥ 27 kg/m²) with at least one comorbidity — type 2 diabetes or pre-diabetes, hypertension, dyslipidaemia, sleep apnoea, non-alcoholic fatty liver disease, joint-loading conditions
- Repeated unsuccessful weight attempts despite lifestyle adjustments
- Type 2 diabetes mellitus with overweight
- Planned surgery where weight reduction lowers peri-operative risk
Procedure flow
- 1First consultation 60 minutes: history, prior findings, life situation, expectations
- 2Laboratory and, where indicated, additional diagnostics
- 3Findings review with individual treatment plan; on indication, private prescription and informed consent on use
- 4Follow-up consultations 30 minutes for therapy guidance, lifestyle support and tolerability
- 5Where needed: handover or integration with endocrinology, obesity surgery, nutritional or sports medicine
Important notes
- This therapy is not a quick fix. Effect and tolerability vary individually; a success guarantee cannot be given.
- Lifestyle components (nutrition, exercise, sleep) remain the foundation. Without them, a lasting effect is unlikely.
- Contraindications are checked at the first consultation — including medullary thyroid carcinoma (including in family history), MEN-2 syndrome, severe pancreatitis history, pregnancy and breastfeeding.
- Possible side effects — mainly gastrointestinal (nausea, fullness, vomiting), more rarely pancreatitis or gallstone-related complications — are discussed in detail.
- Medication costs are borne by the patient (private prescription). Statutory-insurance reimbursement for the obesity indication is generally not provided in Austria.
